GENERAL JOB DESCRIPTION: The IHPS Case Manager is responsible for providing case management services and support for up to five families at a time who have been referred to the IHPS Program. The Case Manager serves as the coordinator of services and supports for each family, overseeing the development and implementation of the IHPS Provider Service Plan and safety plans. Responsibilities Include: • Facilitate case planning meetings with family and IHPS Team, • Provides families with support services that include: o Crisis intervention and stabilization; o Emergency availability; o Ongoing information-gathering in support of active intervention; o Collaborative development and modification of the IHPS Provider Service Plan; o Intervention by each team member with the family in attaining identified IHPS goals and objectives within the service plan, including: linkage and utilization of community-based services and supports, parenting education/support, attainment of concrete goods and supports, monitoring of physical and behavioral health services and school attendance/performance. o Parent education and support for the parents in implementing effective behavior management and parenting approaches specific to the identified needs of the family using the Nurturing Parent Program; o School-based consultation as needed; o Referral, coordination, and linkage to other agencies, social services, and community services, as appropriate; o Assistance in obtaining relief services such as babysitters, homemakers, respite care and supportive services such as transportation and recreation, and developing a network in order to receive these services. • Documents all work; • Performs other related duties as assigned. Knowledge Required: • Knowledge of the principles and practices of social work, child welfare systems and group dynamics as evidence by a Bachelors Degree in social work, human services or a Masters Degree in a related social science field and at least 3 years of experience in these areas. • Knowledge and competence in casework practice, traditional and nontraditional services, skills in negotiation, collaboration and conflict management. • Effective organizational skills, ability to develop interpersonal relationships, and ability to coordinate and/or negotiate with family members, community resource supports and other professionals. • Thorough knowledge of pertinent agency resources, collateral public and private agencies and/or resources and legal policies and procedures that affect children and families. STATUS: Exempt PHYSICAL AND MENTAL REQUIREMENTS: Moderate physical exertion required. Ability to maintain composure in crisis situations with children and adults, ability to restrain youth physically and in accordance with accepted agency guidelines. Computer skills, including knowledge of Word, Excel and database applications. NORMAL WORKING HOURS: Flexible forty-hour workweek. Evening and weekend hours required.
