JOB DESCRIPTION SOCIAL WORKER Department: Residential Administration Qualifications: BSW degree with at least 2 years relevant experience; valid driver’s license. General Job Description: Responsibility for family work assessment, planning and intervention rests with the Social Worker. In practice, this includes having the primary contact with family members, choosing appropriate modalities to treat problems between child and family, and determining and monitoring frequency of visits. The Social Worker is the liaison between Carson Valley School and the referring county and is considered a part of the assigned cottage team. Essential Functions: Family Work Responsibilities: • Meeting with families and children on a planned basis as defined by the treatment plan, regulations and sound clinical practice. • Establishing visiting plan between child and family and monitoring plan. • Documenting all meetings and visits and recording progress and/or changes. • Communicating family issues with cottage team. • Communicating child and family issues with county worker. Cottage Responsibilities: • Meeting weekly with Cottage Leadership. • Participating in all team meetings. • Participating in all Treatment Reviews. • Participates in therapeutic groups and activities as identified by Treatment Team Leader. Case Management: • Maintaining all assigned case records in accordance with State regulations and Agency standards. • Coordinating discharge plans. • Liaison with other agencies that may be involved with child and/or family. • Agency representative to Family Court. Intake Responsibilities • Notifying central intake person of any anticipated openings. • Reviewing referral material. • Obtaining signatures and completing necessary forms to begin case record. • Completing multi-discipline assessment. JOB DESCRIPTION SOCIAL WORKER Page 2 of 2 Additional duties and responsibilities may be assigned when deemed necessary for the overall welfare of the residents and their families and Carson Valley School. Status: Exempt Physical and Mental Requirements: Moderate physical exertion required. Ability to participate in activities with youth. Ability to maintain composure in crisis situations with youth and family. Ability to restrain youth physically and in accordance with accepted Agency guidelines. Normal Working Hours: Flexible. Schedule to be determine based on needs of clients. Some weekend and evening hours required to meet client needs.
